Damage
As ductwork ages, it becomes fragile. Running hoses and/or brushes through them present many opportunities for damage. Cleaning is definitely not recommended with flexible ducts.
Mold
When ductwork sits in a humid environment and combines with poor ventilation, condensation results and mold forms. It cannot be cleaned out of insulated air ducts and will need complete removal and replacement.
What The EPA Says
The Environmental Protection Agency (EPA) states: Duct cleaning has never been shown to actually prevent health problems. Neither do studies conclusively demonstrate that particle (e.g., dust) levels in homes increase because of dirty air ducts. This is because much of the dirt in air ducts adheres to duct surfaces and does not necessarily enter the living space.

The only guarantee that your ductwork is 100% clean is to completely replace it and routinely change out the filters.
